Your Identification card is active and can be used right now by your entire family. Please print your name clearly on the line beside the word “Member”. Cut the card where shown, fold it and present it with your prescriptions and a form of payment at one of our 60,000 participating pharmacies, including most major chains. Ask that all future prescriptions be filled utilizing the program.
Your card does not expire, and is provided to you and your family under the sponsorship of CharityWorks®. Please keep your card in a safe place.
